Using Results from a Social Security Death IndexThe Social Security Death Index brings you valuable information in your genealogic research. However, you can get even more by looking over the original application filed by your ancestor. This was form SS-5. The form includes: * full legal name * full name at birth * mailing address * age at the date of last birthday * date and place of birth * legal names of father and mother * mother´s maiden name * race * sex * whether or not a previous application was filed * name address of current employer * signature * date of signature. To retrieve this information, send a request to: Social Security Administration Office of Earnings Operations FOIA Workgroup 300 N. Greene Street P.O. Box 33022 Baltimore, Maryland 21290 Check on the rates before you send your letter, but at last check they were $27.00 if you know the Social Security number and $29.00 if you do not have the number.
